Job Purpose : This professional position serves as the case manager and service provider to individuals in Employment and Community Services programming. The Case Manager Advanced is responsible for maintaining the client record, communicating with stakeholders, and coordinating the overall care of the person served. The Case Manager also promotes the importance of community integration and inclusion for all through education and service delivery.
In addition , the advanced case manager role indicates the ability to deliver a specialty service based on specific degree or certification, such as Art, Music, Occupational, Horticultural or Recreation Therapy, and/or that the case manager has received training and is participating in ongoing mentoring in administrative and program management responsibilities.
LIFE Services population: individuals with intellectual/developmental disabilities and medical support needs.
Brain Health Services population : individuals with traumatic and acquired brain injuries.
Essential Job Functions :
- Completes all required documentation, meeting established standards and timelines, to uphold the integrity of the client record. Shares pertinent information regarding plan of care and with the team, directs service delivery and monitors progress and goal achievement.
- Maintains effective and supportive relationships and timely communication with stakeholders, including coordinating biannual plan of care conferences.
- Ensures proper authorization is in place prior to service provision and thereafter for length of stay.
- Provides a safe, therapeutic learning environment for all persons served. Programming is focused on enhancing growth and independence through teaching and promoting personal choice, identifying interests and skills sets and developing general ability and function pertinent for community engagement and desired life activities.
- Responsible for assisting individuals at mealtime to ensure safety and promote a pleasant and respectful eating environment.
- Collects pertinent data necessary for metric and outcome reporting.
- Mentors Direct Service Professionals and Program Support Professionals in classroom instruction, engaging persons served, adapting activities, and increasing level of comfort when running groups in the community. Demonstrates professionalism and encourages teamwork.
- Creates new or supplemental components to existing curricula that are relevant, engaging, appeal to different learning styles and modalities, and meets the needs of the individuals served.
- Identifies and introduces assistive technology, accommodations, compensatory strategies, or support plans as needed to improve client presentation and/or performance.
- Represents MCFI and advocates for people with disabilities through formal and informal participation in professional and community organizations and interactions with community partners.
- Practice and promote safety in the workplace; reports all accidents according to agency procedures.
- Perform other related duties as assigned.
Advanced essential functions can include:
- In the absence of the Manager, assists with providing day to day supervision of operations.
- Completes intake and enrollment of new individuals into the Brain Health program.
- Completes audit of client records, ascertains missing information, and tracks plan of care conferences.
- Creates the daily schedule and staff assignments, prints, post and distributes as needed.
- Runs specialized art, music, horticulture or individual and group recreational therapy sessions.
- Responsible for leading daily stand up meeting, taking minutes as needed, reviewing the schedule, and proactively handling any upcoming issues.
Required Technical Skills, Experience, Education, and Credentials :
- A bachelors degree in Community Counseling, Vocational Rehabilitation, Art, Music, Recreation or Occupational Therapy with noted experience in delivering these specialized services OR a Bachelors degree w/ at least 4 years of relevant experience.
- Knowledgeable of the physical, psychological, and social aspects traumatic and acquired brain injury and the resultant impact on the individual and family.
- Knowledge of case management principles and practices.
- Knowledge of community integration and inclusion best practices.
- Must have strong oral and written communication skills and demonstrate computer literacy.
- Skill and ability to interpret observational data to make objective and impartial written/verbal reports.
- Must be comfortable conducting formal classroom sessions and apply various teaching methodologies to meet the needs of the people in programming.
- Understanding of behavior modification techniques, compensatory strategies, and accommodations.
- Current First Aid certification or completed within first 90 days of hire.
- Current Non-Violent Crisis Intervention certification or completed within first 90 days of hire.
- Current negative TB test.
- A personal vehicle, a valid Wisconsin drivers license and insurance in conformity with agency limits.
- Ability and willingness to drive company vans for community-based activities.

Work Environment and Physical Requirements
The work environment and physical demands described here are representative of those that an employee may experience or must do to successfully perform the essential functions of the job:
- This position works in a variety of indoor and outdoor work environments which may include, various outdoor weather conditions, loud noises, and fluctuating temperatures.
- Must be able to safely maneuver a person in a wheelchair on different terrains.
- Must be able to transfer a person from a wheelchair to the toilet, chair, or van seat/car with use of a gait belt and/or manual or power Hoyer lift.
- On occasion, must be able to move, transport, position, push/pull and/or lift objects and equipment weighing up to 100pounds for minimum of 450 feet.
- Must be able to employ Non-Violent Crisis Intervention Techniques and holds.
- Regular city and/or highway driving is required. Use of public transportation may at times be needed.
- This position is responsible for the independent supervision and safety of consumers in a variety of settings in the community. As such, this position must be able to fully observe consumer activities, detect areas of danger, exercise critical thinking skills and enact an emergency plan of action if needed.
