As a Physical Therapist in home health, you will: Conduct comprehensive physical assessments and review medical histories to evaluate patients' conditions. Administer therapeutic exercises and training programs designed to maximize functional independence. Educate patients and their families on injury prevention, wellness, and ongoing health management. Monitor, document, and communicate patients' progress toward their rehabilitation goals. Collaborate with a multidisciplinary team to ensure a comprehensive approach to patient care. Maintain accurate and timely documentation in compliance with healthcare regulations. Qualifications: Masters degree in physical therapy. At least one year of experience as a physical therapist in a home health setting. Current state licensure to practice as a physical therapist. Compliance with accepted professional standards and practices. Valid CPR certification for clinical roles. Valid driver's license with the ability to travel to various locations as needed.
