Travel Physical Therapist - $2,444 per week

Certification Details BLS Job Details Responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home. Work in close coordination with the PACE Physician, rehabilitation staff and other interdisciplinary team members to maximize participant independence and safety and develop or restore function. Collaborate with the Occupational Therapist to efficiently operate WelbeHealth rehabilitation department. Member of the WelbeHealth management team and facilitate collaborative relationships across WelbeHealth and with external stakeholders to promote WelbeHealth values, team culture and mission. Conduct Physical Therapy assessments to determine care needs, preferences and goals of participants. Deliver and document Physical Therapy interventions including active and passive exercise, muscle re-education, gait and functional training of ADLs, and treatments such as massage, heat, sound, water, light or electricity. Supervise Therapy Assistants including providing training, instruction and guidance to implement Functional Maintenance Programs. Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment. Order, track and manage maintenance for equipment. Remain alert to input from team members, participants, and caregivers and update the interdisciplinary team promptly of any changes in participants' condition or functional status. Support smooth care transitions between settings such as hospitals, skilled nursing facilities, and home. Support enrollment of prospective participants into the program in coordination with the Marketing Team. Participate in end-of-life care coordination and support. Manage direct reports including training, supervising and mentoring. Complete all required documentation in a timely and accurate manner. Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families. Follow WelbeHealth policies and procedures and participate in required Quality Improvement activities, staff training and meetings. Communicate regularly with Supervisor and team regarding workload and priorities. Timely completion of all mandated trainings and education. Timely completion of all mandated occupational health screenings as needed. Exercise flexibility in performing assignments as business needs evolve. Job Requirements Five years of progressively responsible Physical Therapy experience. Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population. Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs. Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations. Two years supervisory experience with demonstrated ability to mentor and develop team members. Ability to process accurate information in a timely manner. Excellent leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team members. Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects. Highly motivated, inspirational leader who can innovatively engage, support and remove barriers for the team to ensure goals are met.
