Per Diem Physical Therapist Type of Work: PRN or Per Diem Set up : SNF (Skilled Nursing Rehabilitation) Rate: $55 & Weekends $57 Location: Los Angeles, CA (30 miles Radius - LA metro) Requirement: 1. Graduate of an American Physical Therapy Associations accredited Physical Therapy degree program Experience and Licensing Requirements: 1. California State Physical Therapy License 2. Current CPR certification. 3. Minimum 1 year working in SNF setting and experience in Net Health 4. Authorization to work in the United States. 5. Demonstrated ability to work independently and collaboratively in a fast-paced environment. Job responsibilities: 1. Conduct thorough assessments to evaluate patients' physical and functional status. 2. Develop individualized treatment plans based on assessment findings and patients' goals. 3. Implement evidence-based therapeutic interventions to promote mobility, strength, and function. 4. Monitor patients' progress and modify treatment plans as necessary to achieve desired outcomes. 5. Educate patients and their families on exercises, techniques, and strategies for self-management and injury prevention. 6. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans. 7. Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and ensure continuity of services. 8.
