Cancer Detection Program Case Manager (Chula Vista, CA - Onsite)

Position Summary:
The Case Manager will be responsible for providing case management under the Cancer Detection Program and other departments as needed. General responsibilities will be, but are not limited to: attending staff meetings and trainings, providing coverage for case management staff, interfacing with clinical staff, providing appropriate referral and follow-up for abnormal screenings, and attend trainings as requested by management. The position is to ensure that at all levels, women get proper services and follow-up through the entire process. The Case Manager must maintain confidentiality at all levels.
Essential Functions of the Job:
- Conducts Case Management duties to include:
- Tracks women and provide referrals and follow-up.
- Documents all case management and follow-up in charts
- Enrolls women who need treatment in BCCTP or appropriate treatment program,
- Provides diagnosed patients with appropriate referrals to cancer navigation programs.
- Ensures patients are tracked and return for breast and cervical cancer screening in time frame as instructed by provider and protocol. Updates care guidelines.
- Checks women for eligibility and enroll in appropriate programs (Every Women Counts, EWC).
- Provides Case Management duties for patients to ensure adherence to all CDP and SYHealth protocols and standards.
- Provides breast and cervical cancer education and outreach as requested.
- Maintains program specific records and logs as required in all sources, including DETEC state data base system.
- Maintains client case management and productivity records and submits tabulated data monthly.
- Provides coverage at various sites as needed.
- Attends meetings and/or training sessions as required and participates on committees as directed.
Additional Duties and Responsibilities:
- Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
- Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
- Performs other related duties as assigned or requested

Job Requirements:
Education Required:
High School Diploma or equivalent.
Certifications/Licenses Required:
CA driver's license with appropriate insurance coverage.
Experience and Required Skills:
- Medical background and experience with case management, referral management, and/or case coordination
- Familiarity with, and advanced knowledge of, community resources and their use to meet the needs of families.
- Has organization and project management skills.
- Advanced knowledge of the health care delivery system.
- Has appropriate knowledge of general health, specifically cancer related information.
- Ability to establish and maintain good interpersonal relationships.
- Proven communication and writing skills.
- Ability to work independently and meet deadlines.
- Experience and knowledge with Microsoft Office software.
Verbal and Written Skills Required to Perform the Job:
Bilingual (Spanish/English). Demonstrated ability to communicate effectively to adolescents, caregivers and others. Demonstrates excellent oral and written communication skills.
Equipment Used:
Computer, phone, and related office equipment.
Working Conditions and Physical Requirements:
Physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have full range of motion; ability to lift 30-50 lbs; T.B. clearance. Busy clinic with medium to loud noise levels and controlled office temperatures. Frequent standing and walking indoors and outdoors. Could be required to travel to other sites or other locations in San Diego County. Could be required to work some evenings and/or weekends.
