Position Summary

The position is primarily responsible for providing rehabilitation services, but is also responsible for providing all aspects of physical therapy services, including but not limited to screening, examination, evaluation, care planning, treatment, reassessment, discharge planning and education to adults through geriatric patients.

Essential Functions and Responsibilities of the Job

• Communicate effectively with all the disciplines involved in the patients’ care to facilitate a better understanding of each patient’s status and to effectively provide essential information to other disciplines or staff members.
• Complete charting and billing daily.
• Manages time to complete all daily tasks.
• Establish goals and a plan of care in collaboration with patient and family.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Qualifications/Work Experience\:

• One-year clinical physical therapy experience required. In lieu of this experience, consideration with be for staff with strong education and associated experiential background.
• Knowledge of applicable Title 22 regulations, JCAHO standard and Physical Therapy Practice Act regulations.

Education/Licensure/Certification\:

• Current BCLS-C Certification
• Current CA Physical Therapy License
