About the Role: Bachelor Mental Health Counselor: Star Rise

Star Rise is a brand-new, 58-unit Permanent Supportive Housing (PSH) community in Greeley, designed with trauma-informed principles to promote healing, safety, and stability for individuals on the path to recovery. With thoughtfully crafted spaces—from bright corridors to peaceful outdoor decks, Star Rise offers more than just housing; it’s a foundation for new beginnings.

As a Bachelor Mental Health Counselor, you’ll be a key part of a dynamic, compassionate team committed to delivering trauma-informed, evidence-based care. Working closely with clinicians and the Program Director, you'll support individuals in navigating mental health and substance use recovery. From conducting assessments to crisis intervention, your role will help create a seamless care experience that empowers clients to thrive.

Key Responsibilities:

  • Ensure compliance with utilization, quality improvement, and corporate policies and standards.
  • Provide direct services to clients as assigned by your supervisor, including assessments, goal setting, and treatment for mental health challenges.
  • Conduct screenings, intakes, and assessments within established timelines, utilizing the required tools and formats.
  • Develop and update strength-based treatment plans and continuing care plans, ensuring timely referrals and discharge summaries.
  • Assess clients’ diagnoses and mental status and create individualized treatment plans to meet their needs.
  • Provide crisis intervention and de-escalation in various settings and assess risk to ensure client safety.
  • Collaborate with clinicians, law enforcement, external providers, and psychiatrists to deliver coordinated care.
  • Work with clients on safety planning, symptom management, and risk education to enhance support systems and improve overall well-being.
  • Utilize therapeutic interventions and behavioral skills training to help clients develop coping strategies, make informed decisions, and solve daily life challenges.
  • Support and implement evidence-based practices to ensure the effectiveness of client interventions.
  • Provide case management and connect clients to internal and external resources to support treatment goals.

Qualifications:

  • Bachelor’s degree in psychology, social work, or a related field is required.
  • Valid driver’s license and a driving record that meets insurability requirements.
  • Successful completion of background, fingerprinting, and drug testing
  • Bilingual (English/Spanish) skills are desired.

Experience:

  • One year of experience working with individuals with mental health disorder challenges.
  • Experience in coordinating behavioral health intervention plans and services with our agencies is preferred.

  • Annual Flu Vaccinations
