The Patient Care Coordinator develops, implements, monitors and evaluates standards of care for gynecology and obstetrics surgery patients of Women's Healthcare Associates, LLC. Acts as an advocate for WHA patients and as a liaison between patients and the hospital. Functions as primary contact for hospital pre-admit and Labor and Delivery staff. Provides support to the patient from the beginning of their surgery journey to the end.
DUTIES
- Schedules surgery, re-evaluation appointment and first post-operative appointment. Speaks with surgery patients prior to surgery, reviews details of scheduled procedures and answers patients' questions.
- Discusses instructions for pain management and medications with patients as needed.
- Makes contact with patient post-surgery to answer questions, provide support and information.
- Contacts clinician via Outlook invite with date, time, and other information when surgeries are
scheduled no less than one week prior to the surgery.
- Notifies Patient Account Specialists to obtain prior authorization from the health insurance for the surgery.
- Makes arrangements for special needs such as physical assessments, notifying hospital of need for interpreter, special equipment for obese or disabled patients, caregivers who will be primary contact for patient, etc.
- Assists in coordinating add-on surgeries when indicated.
- Schedule patients' post operative appointments.
- Makes contact with and coordinates surgeries with surgeons outside WHA (For example, GYN oncology cases, concurrent procedures such as breast surgery, hernia repair, etc.)
- Assists in transfer of care to other WHA campuses when patient needs to utilize a different hospital.
- Maintains cooperative relationships with clinicians, clinical and patient services staff as well as hospital staff in coordination of patient care.
- Reviews and maintains various logs and records
Qualifications
Education & Experience
- Certification from an accredited medical assisting program preferred, or the equivalent
combination of classroom training and/or work experience; or graduate of an accredited school of Nursing and a valid licensure as a Licensed Practical Nurse or Registered Nurse in the State of Oregon.
- At least two years of experience in a gynecology medical setting required, five years preferred.
Additional Licenses, Certifications and/or Specialty Requirements
CPR certification required.
