- Empower patients and their families by educating them about therapeutic goals, safe discharge planning, and home exercise routines.
- Maintain meticulous documentation of assessments, treatments, and patient progress in line with facility protocols and regulatory requirements.
- Participate actively in interdisciplinary care team rounds and conferences to ensure a seamless continuum of care.
- Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.
- Valid state licensure as a Physical Therapist .
- CPR certification is preferred.
