Physical Therapist Position Highlights:

Setting: Onsite Required ( No Remote, No Telehealth, or No Hybrid) - 5 Star Medicare Skilled Nursing Facility

Competitive Pay: $60-$65 per hour (approx.$124,800 - $135,200 Annual Salary)

PTO: 2 weeks of PTO per year + Paid holidays

Patient Type: Adult and Geriatric Patients

EMR System: Net Health

Flexible Schedule: Monday - Friday, Sunday - Thursday, or Tuesday - Saturday

Shift/ Hours: 8:00 am -5:00 pm - 5x 8s - 40 hours per week

Responsibilities:

Conductprehensive wound assessments and develop individualized treatment plans in collaboration with physicians and interdisciplinary teams.

Perform Wound Mist Therapy treatments inpliance with state regulations and facility protocols.

Implement evidence-based wound care interventions, including offloading,pression therapy, debridement rmendations, and infection control measures.

Maintain accurate and timely documentation in accordance with Medicare, Medicaid, and CA state healthcare regulations.

Educate patients, caregivers, and facility staff on wound care best practices and prevention strategies.

Collaborate with physicians, nurses, dietitians, and other allied health professionals to ensureprehensive patient care.

Monitor treatment efficacy and wound healing progress, making necessary adjustments to treatment plans.

Ad to HIPAA guidelines and maintain patient confidentiality.

Stay up to date with current wound care advancements and participate in ongoing professional development.

Qualifications & Requirements:

1 or more years of Physical Therapy experience preferred

New Grad Friendly - Mentorship Available - Active CA PT License Required

Active California Physical Therapist (PT) License Required - Must be in good standing with no disciplinary actions

Active BLS Card Required - American Heart Association or American Red Cross

Minimum of 1 year of experience in wound care, rehabilitation, or skilled nursing facility (preferred but not required).

Certification in wound care (e.g., WCC, CWS, CWPT) is a plus.

Knowledge of Medicare/Medicaid guidelines for wound care services in skilled nursing facilities.

Ability to work independently and within an interdisciplinary team.

Strong clinical reasoning, patient education, and documentation skills.

Familiarity with electronic medical records (EMR) systems.
