We are seeking a passionate and skilled Physical Therapist Assistant to join our team on a per diem basis, providing exceptional care directly in patients' homes in Santa Clara, CA. This role requires a dedicated professional who is committed to improving patients' quality of life through personalized physical therapy services.

Key Responsibilities

Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.

Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.

Collaborating with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.

Required Qualifications

Current Physical Therapist Assistant license in the State of CA.

Valid CA Fingerprint Clearance Card.

Up-to-date BLS (CPR) / ACLS certification.

Recent Negative TB test result and Flu Shot within the past year.

Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.

Valid Driver's License along with auto-insurance.
