Position Overview: As a Physical Therapist at Santa Rosa Post Acute, you will be responsible for assessing and treating patients to help them regain physical function and improve mobility. Working within a collaborative healthcare team, you'll design and implement individualized treatment plans that promote independence and enhance the quality of life for our residents. Key Responsibilities: Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans. Provide physical therapy interventions to improve strength, mobility, and functional abilities. Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications. Collaborate with the interdisciplinary team, including occupational therapists, nurses, and physicians, to coordinate care and optimize patient outcomes. Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes. Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies. Ensure that care plans align with regulatory requirements and best practices in physical therapy. Qualifications: Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program. Valid Physical Therapy license in [State]. Experience working in a skilled nursing facility or similar healthcare setting preferred. Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment.
