TITLE: CASE MANAGER
STATUS:EXEMPT
SPECIALPOPULATION served by Case Manager:
SMI Children/Adolescent Substance Abuse DD
LINESOF AUTHORITY: This position is supervised by the ProgramDirector or their Designee. Unlessspecifically designated this position does not supervise other personnel. The position may require periodicparticipation in peer review activities.
POSITIONSUMMARY: Advocate for clients, obtain entitlements, coordinate transportationbetween ChangePoint and other service providers. The BHT/Case Manager is expected to cooperateas a team member in the provision of a therapeutic environment that encourageseach customer's emotional growth. Thisposition will be expected to maintain an acceptable level of productivity asmeasured by billable hours. This positonis expected to meet all standards for clinical documentation.
PRINCIPLEDUTIES/RESPONSIBILITIES:
Adheres to personnel policies andcomply with agency requirements for ethical and professional behavior.
Provides services to clients as a casemanager.
Is responsible for all aspects of theclinical records, provides crisis services and assist with service coordinationfor the client.
Engages and includes the client in thedevelopment and implementation of the treatment plan.
Protects client confidentiality andpersonal rights.
Keeps clear, concise and accuraterecords of clinical activity according to Policy and Procedures.
Assists clients in obtainingentitlements and benefits pertaining to housing, income, health and othersupportive services, as needed
Provides recreation, leisure , andpro-social skills for clients both individually and in groups
Provides transportation as needed.
Workwith other agencies to provide wrap services including going in the home and/orschool to prevent out of home placement
Providesassertive outreach to clients who resist involvement or have difficultyaccessing services
Establishesand maintains a working relationship with personnel and referral resources.
Providevocational services:
Perform vocational screenings andevaluations
Act as a liaison between client,Department of Vocational Rehabilitation, and employer.
Provide Client advocacy
Provide job development and placementservices
Provide community training sitedevelopment
Providehealth promotion.
Coordinates and accompanies clients toPCP/Doctor appointments.
Advocates for client at Doctorappointments
Provides health promotion groupsincluding but not limited to hygiene, exercise, and nutrition.
Mustmeet state standards for continued education.
If qualified, providesrotating on-call coverage and update tx plans
Thisposition requires positive and active support of the company, the client andthe co-worker.

Mustparticipate in quality management and quality improvement activities.
Performsother duties as assigned by their supervisor or designee.
PRIMARYCONTACTS:
Frequent interaction with clients and theirsupport system, usually their family.
Interfaceas needed with peers and other clinical team's members.
Collaboration,as appropriate, with other community resources/referral agencies, with thecustomer's consent.
High School Diploma
plusfive (5) years Behavioral Health Experience for BHT case management level
CurrentArizona Driver's License.
FingerprintClearance.
Complywith a Drug Free workplace.
Must be at least 21 years old.
Knowledge and ability to define theneeds of the client served.
ForThose Working With SMI
1. Knowledge ofsymptoms related to qualifying diagnosis.
CPRand First Aid certification.
Knowledgeof entitlement process and local resources.
Fluentin English, both verbally and in writing.
Literate.
Ableto effectively employ analytical and problem-solving skills.
Able to prioritize, handle multipletasks with customer.
Abilityto accurately complete difficult paperwork in a timely manner
producing professional documents, charting incustomers charts, and record
keeping using TJC, NARBHA and DHS standards.
Experienceand training in crisis intervention.
Familiarwith psychotropic drugs and their side effects.

development with childhood disorder.
2. Knowledgeof Behavior Management and ability to develop age-
appropriate interventions.
3. Able to communicate at an age-appropriatelevel.
ForThose Working With Substance Abuse:
1. Able to identify signs and symptoms ofSubstance Abuse and Chemical
Dependency.
2. Knowledge of range of treatment fordependence including relapse,
prevention, strategies and self helpgroups.
3.Able to provide a diagnostic impression per DSM IV criteria for Substance
Abuse and Chemical Dependency.
4. Knowledgeof history of substance dependence and of the
biopsychosocial influences and effects ofdependencies.
ForThose Working With DD:
1. Knowledge of symptoms related to qualifying diagnosis.
2. Ability to identify and communicate signs ofregression and deterioration.
3. Knowledge of Article 9 (customer rights) ofState Requirements.
4. Knowledge of Customer Intervention Techniques relative to self care and
ADL skill development.
