Speech Therapist

SUMMARY:

Under the direction of the Director of Clinical Services, the Speech-Language Pathologist (SLP) provides comprehensive speech-language pathology services to geriatric participants of the PACE program. Services are delivered in the PACE Center, nursing and assisted living facilities, and participant homes. The SLP is responsible for evaluation, treatment, education, and collaboration with the interdisciplinary team to optimize participants communication, swallowing, and cognitive-linguistic functioning. This role ensures services meet all state, federal, and PACE regulatory requirements and aligns with the organizations mission, values, and standards.

ESSESNTIAL DUTIES AND RESPONSIBILITIES:

  • Provide speech-language pathology evaluations, treatments, and interventions in the PACE Center, nursing/assisted living facilities, and participants homes.
  • Complete initial, periodic, and as-needed reassessments; develop and update individualized treatment plans as part of the interdisciplinary plan of care.
  • Observe participants for changes in physical, cognitive, communication, and social functioning; report changes promptly to the appropriate licensed nurse or team member.
  • Document all evaluations, treatments, and participant responses in the electronic medical record in accordance with professional, regulatory, and PACE documentation standards; ensure notes are signed by the speech-language pathologist.
  • Participate actively in interdisciplinary care planning meetings and collaborate with team members to coordinate 24-hour care delivery.
  • Provide caregiver and staff training on communication strategies, safe swallowing techniques, and adaptive communication devices as appropriate.
  • Participate in quality improvement initiatives by collecting and analyzing outcome data, reviewing treatment protocols, and contributing to program development.
  • Demonstrate sound clinical reasoning, integrating complex evaluation data to establish accurate diagnoses and comprehensive, participant-centered plans of care.
  • Work flexible hours as necessary to meet participant needs in alignment with individualized care plans and departmental staffing requirements.
  • Participate in quality improvement initiatives.
  • Maintain compliance with infection prevention protocols, HIPAA regulations, and all applicable laws and regulations.

EXPERIENCE AND EDUCATION:

  • Masters Degree in Speech-Language Pathology required.
  • Licensed by state Board Examiners in Speech-Language Pathology and Audiology.
  • Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
  • 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring.

PRE-EMPLOYMENT REQUIREMENTS :

  • All Employees - Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
