Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor of Science in Physical Therapy from an APTAapproved program, or equivalent Masters/Doctoral degree in Physical Therapy.
• Current, active Tennessee Physical Therapist license required prior to start.
• Current CPR certification required.
• Current valid drivers license, reliable transportation, and personal auto insurance required.
• 12 years of experience as a Physical Therapist in acute care or rehabilitation settings required.
• Home health experience preferred.
• Ability to perform patient assessments, develop and revise Plans of Care, document progress notes and other clinical records per agency and physician direction.
• Ability to instruct patients and families in transfer techniques and safe use of durable medical equipment such as walkers, wheelchairs, canes, braces and prosthetic devices.
• Ability to lift, bend, push, pull, and perform prolonged standing and walking as required for home visits.
• Ability to work a flexible schedule and travel locally as needed.

• Collaboration: Work as part of an interdisciplinary team and consult with physicians as needed.
• May require local travel between patient homes. Candidate must have reliable transportation and appropriate insurance.
