Minimum Requirements
- Bachelor of Science in Physical Therapy from an APTAapproved program, or equivalent Masters/Doctoral degree in Physical Therapy.
- Current, active Tennessee Physical Therapist license required prior to start.
- Current CPR certification required.
- Current valid drivers license, reliable transportation, and personal auto insurance required.
- 12 years of experience as a Physical Therapist in acute care or rehabilitation settings required.
- Home health experience preferred.
- Ability to perform patient assessments, develop and revise Plans of Care, document progress notes and other clinical records per agency and physician direction.
- Ability to instruct patients and families in transfer techniques and safe use of durable medical equipment such as walkers, wheelchairs, canes, braces and prosthetic devices.
- Ability to lift, bend, push, pull, and perform prolonged standing and walking as required for home visits.
- Ability to work a flexible schedule and travel locally as needed.

- Clinical focus: Evaluation and treatment of neuromuscular, neurological and orthopedic diagnoses across the lifespan.
- Documentation responsibilities: Prepare and revise Plan of Care, progress notes, and complete phone order documentation as required.
- Collaboration: Work as part of an interdisciplinary team and consult with physicians as needed.
- May require local travel between patient homes. Candidate must have reliable transportation and appropriate insurance.
