Human Touch Home Health is hiring a licensed Physical Therapist to provide home health services in Warrenton, Culpeper, Fauquier County, Culpeper County, and surrounding communities.

• Complete physical therapy evaluations, reassessments, and discharge planning
• Develop individualized treatment plans and provide skilled therapy services in patients homes
• Help patients improve strength, balance, mobility, and independence
• Coordinate care with physicians and the interdisciplinary team
• Complete documentation accurately and on time
