• Active California Physical Therapist license
• Previous experience in skilled nursing facilities preferred; mentorship available for those with 0-1 years of experience
• Strong skills in documentation and effective time management
• Flexible, collaborative, and patient-centered approach to care
• Willingness to float between nearby sister facilities as needed
• Schedule flexibilitypreference given to candidates able to work SundayThursday or TuesdaySaturday

Responsibilities include:

• Conducting comprehensive evaluations and implementing individualized treatment plans tailored to each patients needs
• Delivering evidence-based therapeutic interventions to improve resident outcomes
• Collaborating closely with the interdisciplinary care team to ensure cohesive, holistic treatment
• Documenting evaluations, progress notes, and discharge reports using Net Health in accordance with facility standards
