Qualifications:

• Valid California Physical Therapist license
• Prior experience in skilled nursing facilities preferred; mentorship provided for candidates with 01 year of experience
• Excellent clinical documentation and time management abilities
• Strong teamwork and adaptability, with openness to learning and collaborating
• Willingness to work at a nearby facility within walking distance if needed

Primary Responsibilities:

• Conduct patient assessments and develop individualized treatment plans
• Deliver evidence-based physical therapy interventions to improve functional outcomes
• Work closely with the interdisciplinary team, including nurses and occupational/speech therapists, to coordinate care
• Complete and maintain clear, accurate documentation using Net Health EMR per facility protocols
