Schedule/Hours:

• Full-time, 5 x 8-hour shifts
• Includes every other Saturday
• Weekend, holiday, and on-call requirements as needed
• Floating assignments as required

Qualifications:

• Graduate of an APTA-accredited Physical Therapy program
• Active PT license and BLS certification
• Previous hospital therapy experience preferred
• Adaptability for varied caseloads and assignments

Key Responsibilities:

• Evaluate functional abilities and limitations of patients in home health and hospital settings
• Develop and implement individualized treatment plans considering each patients unique physical, social, and cultural needs
• Document care and progress according to regulatory and professional guidelines
• Facilitate smooth patient transitions by communicating vital information within the care team
• Supervise support personnel to ensure safe and effective patient care
• Participate in departmental operations and meetings
