Home Health Occupational Therapist (OT) - Rolla, Missouri

Deliver vital in-home therapy and drive patient-centered outcomes as a Home Health Occupational Therapist. Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Approximately 2532 points per week (about 57 patient visits daily)

Key Responsibilities:

• Complete comprehensive OT evaluations, including OASIS assessments for varied patient needs.
• Create and execute detailed, individualized treatment plans focused on ADLs, IADLs, cognition, upper extremity function, and home safety.
• Deliver skilled interventions such as therapeutic activities, self-care/lifestyle adaptation, adaptive equipment, environmental modifications, and energy conservation.
• Evaluate and recommend appropriate durable medical equipment.
• Monitor patient progress, revise care plans as needed, and update collaborating team members.
• Educate patients and caregivers on safety, adaptive techniques, and functional independence.
• Accurately document all patient interactions and assessments using Home Care Home Base (HCHB).
• Engage in interdisciplinary meetings, case conferences, and ongoing professional growth opportunities.
• Ensure compliance with all regulatory and agency policies.

Desired Qualifications:

• Licensed Occupational Therapist (OT) in good standing in the state of practice
• Minimum one year of recent home health OT experience (field-based experience strongly preferred)
• Familiarity with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and auto insurance
• Strong assessment, organizational, and communication skills; ability to self-manage in the field
