Home Health Occupational Therapist - Saint James, MO

A rewarding opportunity is available for an experienced and compassionate occupational therapist with a focus on home health to deliver exceptional care in patients' own environments. The ideal professional brings expertise in assessing needs, crafting personalized care plans, and fostering greater independence for individuals recovering or coping with physical or cognitive limitations.

Schedule:

• Full-time contract, 13 weeks
• Monday Friday, 8:00 AM 5:00 PM
• Approximate caseload: 2532 points per week (typically 57 patient visits per day)
• Travel required within assigned territory (mileage reimbursement provided)

Qualifications:

• Active Occupational Therapist (OT) license in good standing for the state of practice
• At least 1 year of recent experience in Home Health Occupational Therapy (field experience strongly preferred)
• OASIS assessment experience required
• Familiarity with Home Care Home Base (HCHB) documentation preferred
• Reliable transportation, valid drivers license, and proof of auto insurance
• Excellent communication, organization, and time management skills
• Ability to work independently in a field-based setting

Key Responsibilities:

• Conduct comprehensive OT assessments, including OASIS for required start of care, resumptions, recertifications, and discharges
• Develop individualized treatment plans centered on activities of daily living, safety, upper extremity function, cognition, and adaptive equipment use
• Provide skilled interventions, therapeutic activities, and patient/caregiver education on safety, independence, and compensatory techniques
• Maintain accurate and timely documentation in HCHB
• Collaborate with a multi-disciplinary team, attend case conferences, and participate in ongoing professional development
• Adhere to all agency, regulatory, and Medicare guidelines
