Home Health Occupational Therapist in Dixon, MO

Seeking a compassionate and skilled clinician to provide home-based occupational therapy services in a supportive environment. This contract opportunity offers the chance to make a meaningful impact on patients' independence and safety within the comfort of their own homes. Your expertise will help clients improve their ability to perform activities of daily living (ADLs) and overall function.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• Full-time, 13-week contract
• Caseload of approximately 2532 points per week (typically 57 visits per day)
• Daily travel within the assigned territory; mileage reimbursement included

Key Responsibilities:

• Conduct comprehensive occupational therapy evaluations, including OASIS assessments for applicable visits
• Create and implement individualized care plans focused on ADLs, upper extremity therapy, cognition, and home/environmental safety
• Deliver skilled interventions: therapeutic activities, self-care and energy conservation training, adaptive equipment use, and environmental modifications
• Assess and recommend appropriate durable medical equipment (DME)
• Monitor patient progress, modify treatment plans as necessary, and maintain clear communication with physicians and interdisciplinary team members
• Educate patients and caregivers regarding adaptive techniques, functional independence, and safety in the home environment
• Accurately document care in Home Care Home Base (HCHB) and participate in case conferences and interdisciplinary meetings

Qualifications:

• Active Occupational Therapist licensure in good standing for the state of practice
• Minimum of 1 year recent Home Health OT experience
• OASIS and field experience required; HCHB proficiency preferred
• Strong clinical, assessment, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
