Deliver exceptional rehabilitation services and help individuals regain independence as a Home Health Occupational Therapist. In this field-based position, you will visit patients in their homes, utilizing your clinical expertise to assess needs, develop tailored treatment plans, and implement evidence-based interventions. Your compassionate care will directly support patients goals to improve safety, mobility, and overall quality of life.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points weekly (typically 57 patient visits per day)
- 13-week contract
Key Responsibilities:
- Conduct occupational therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge.
- Develop and execute individualized plans of care focused on activities of daily living (ADLs), upper extremity function, cognition, and environmental adaptations for home safety.
- Deliver skilled interventions: therapeutic activities, energy conservation, self-care and adaptive equipment training, and environmental modifications.
- Recommend durable medical equipment following patient assessments for optimal independence.
- Track patient progress, adjust therapy plans, and collaborate with physicians and interdisciplinary teams.
- Educate patients and caregivers on functional independence, use of adaptive equipment, and safety strategies.
- Maintain detailed and timely documentation using Home Care Home Base (HCHB).
- Participate in case conferences, interdisciplinary meetings, and ongoing professional development activities.
- Ensure adherence to agency policies, regulatory standards, and Medicare guidelines.
Qualifications:
- Active Occupational Therapist license in the state of practice (in good standing)
- Minimum 1 year of recent Home Health OT experience (field settings preferred)
- Competency with OASIS documentation; HCHB experience preferred
- Reliable transportation, valid drivers license, and auto insurance
- Strong independent work ethic, clinical, and communication skills
