As a dedicated clinician, you'll foster independence, improve mobility, and support optimal recovery through evidence-based practices and compassionate care.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: ~2532 points weekly (57 home visits daily, based on visit types)
• Field-based setting, daily travel within assigned territory (mileage reimbursement provided)

What You'll Do:

• Complete comprehensive physical therapy evaluations, including OASIS assessments for various care phases.
• Develop and carry out individualized treatment plans targeting mobility, balance, strength, gait, pain management, and safety.
• Provide skilled therapy interventions such as therapeutic exercise, neuromuscular re-education, fall prevention, and transfer training.
• Educate patients and caregivers on safety strategies, assistive device usage, and home exercise routines.
• Collaborate with interdisciplinary healthcare professionals to ensure holistic, coordinated care for each patient.
• Communicate patient status and adapt plans as necessary to maximize outcomes.
• Accurately document all care activities and progress using Home Care Home Base (HCHB).

Qualifications:

• Active Physical Therapist (PT) license in good standing for the state of practice
• Minimum 1 year of recent home health physical therapy experience
• OASIS experience required; proficiency with Home Care Home Base (HCHB) preferred
• Demonstrable clinical decision-making and functional assessment skills
• Strong communication, time management, and organizational abilities
• Reliable transportation with valid drivers license and insurance
• Ability to operate independently in a field-based role
