Home Health Physical Therapist (PT) - Forrest City, AR

Step into a fulfilling opportunity providing vital rehabilitation services to patients in their homes as a Home Health Physical Therapist. In this contract position, you will make a meaningful impact by guiding individuals toward increased strength, mobility, and independenceall while collaborating closely with a supportive interdisciplinary team.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload of approximately 2532 points per week (57 patient visits per day, dependent on visit type)

Desired Qualifications:

• Active and unrestricted Physical Therapist (PT) license in state of practice
• At least 1 year of recent Home Health physical therapy experience (field-based strongly preferred)
• Confident utilizing OASIS for assessment (Start of Care, Resumption, Recertification, Discharge)
• Familiarity with Home Care Home Base (HCHB) for documentation preferred
• Clinical expertise in developing and adjusting functional treatment plans
• Effective communication, time management, and organization abilities
• Valid drivers license, reliable transportation, and current auto insurance

Key Responsibilities:

• Conduct comprehensive patient assessments, including required OASIS evaluations
• Design and implement tailored therapy programs to support mobility, balance, strength, pain control, and safety
• Deliver skilled interventions such as therapeutic exercise, gait and transfer training, and fall prevention strategies
• Update treatment plans based on patient progress and collaborate with physicians and team members
• Educate patients and caregivers about exercises, safety, and adaptive equipment
• Complete timely and accurate documentation in HCHB
• Attend interdisciplinary meetings and ongoing education
