Home Health Physical Therapist in Georgetown, KY

Experienced and compassionate professionals with a background in home health physical therapy are invited to provide high-quality care to patients in a field-based setting. This position, offered on a full-time, 13-week contract, is ideal for those skilled at working independently and delivering patient-centered therapy that promotes improved mobility, strength, and independence.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Responsibilities:

• Complete comprehensive physical therapy evaluations, including OASIS assessments as required.
• Develop and implement individualized plans of care focused on functional goals such as balance, gait, and strength.
• Deliver skilled interventions: therapeutic exercises, gait and transfer training, neuromuscular re-education, and fall prevention.
• Assess patient progress, update care plans as needed, and communicate with physicians and interdisciplinary partners.
• Educate patients and caregivers in safety, exercise, assistive device use, and home exercise programs.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate closely with RNs, OTs, SLPs, medical social workers, and aides to ensure comprehensive care.
• Participate in meetings and case conferences and stay current with best practices and agency policies.

Qualifications:

• Active Physical Therapist (PT) license in the state, in good standing
• At least 1 year of recent home health PT experience (field work preferred)
• OASIS assessment experience required
• Familiarity with HCHB strongly preferred
• Reliable transportation, valid drivers license, and auto insurance essential
• Strong assessment, clinical decision, and organizational skills
• Ability to manage a flexible, field-based schedule independently
