Physical Therapist Home Health Nicholasville, Kentucky

Deliver essential rehabilitation in patients homes and help them reclaim independence as a Home Health Physical Therapist. This contract opportunity centers on providing individualized and outcome-focused physical therapy services directly in the home setting. Youll work autonomously and as part of a broader interdisciplinary care team, making a lasting impact on patient mobility, safety, and quality of life.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week engagement
• Daily travel to patient residences (mileage reimbursement provided)
• Approximately 2532 points per week (57 patient visits daily, based on visit type)
• Patient documentation using Home Care Home Base (HCHB); assessments utilizing OASIS

Key Responsibilities:

• Complete comprehensive home health physical therapy evaluations, including OASIS for various care phases
• Develop and implement tailored treatment plans emphasizing functional mobility, strength, gait, balance, and fall prevention
• Provide hands-on interventions including therapeutic exercises, transfer training, neuromuscular re-education, and gait training
• Track patient progress, update plans as appropriate, and keep physicians and care team informed
• Educate patients and caregivers on exercises, home safety, and use of assistive devices
• Collaborate closely with nursing, OT, SLP, medical social work, and home health aide team members to coordinate care
• Maintain accurate records within HCHB, attend interdisciplinary meetings, and uphold regulatory standards

Qualifications:

• Active and current Physical Therapist (PT) license
• Minimum 1 year of recent home health physical therapy experience; field-based experience strongly favored
• OASIS experience required; HCHB familiarity preferred
• Strong problem solving, organization, and communication skills
• Reliable vehicle, valid drivers license, and auto insurance
