A skilled Physical Therapist is needed to provide compassionate home health services. This opportunity allows you to make a significant impact by delivering care to patients in the comfort of their own homes, with a focus on improving mobility, function, and independence.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive in-home physical therapy evaluations, including required OASIS assessments.
- Create and execute individualized treatment plans focusing on mobility, strength, balance, gait, pain management, and patient safety.
- Deliver hands-on therapy including therapeutic exercises, neuromuscular re-education, gait training, transfer techniques, and fall prevention.
- Monitor and document patient progress, adapting plans and communicating changes to physicians and the healthcare team as needed.
- Educate patients and caregivers regarding home exercise routines, safety precautions, and assistive device usage.
- Maintain meticulous, timely documentation using Home Care Home Base (HCHB).
- Collaborate effectively with other disciplines such as nursing, occupational and speech therapy, and social work.
- Attend case conferences, interdisciplinary meetings, and participate in ongoing education.
Desired Qualifications:
- Active, unrestricted Physical Therapist (PT) license in the relevant state.
- At least 1 year of recent home health physical therapy experience; field experience highly valued.
- Proficiency in the use of Home Care Home Base (HCHB) and OASIS assessments.
- Demonstrated organizational, time management, and strong clinical decision-making abilities.
- Reliable transportation, valid driver's license, and auto insurance.
- Ability to thrive both independently and as a collaborative team member.
