Home Health Physical Therapist (PT) in Greenville, NC

Deliver skilled and compassionate care as a Home Health Physical Therapist, providing crucial physical therapy services to patients in their homes. Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough physical therapy evaluations utilizing OASIS assessments as required
• Create and oversee patient-centered care plans aimed at improving mobility, strength, and independence
• Administer interventions such as therapeutic exercise, neuromuscular re-education, and fall prevention strategies
• Monitor patient progress, adjusting treatment plans, and promptly communicating any status changes to the care team
• Educate patients and caregivers on exercise techniques, safety protocols, and use of assistive devices
• Accurately document all care in Home Care Home Base (HCHB)
• Collaborate closely with other healthcare professionals to coordinate comprehensive patient care
• Participate in interdisciplinary meetings and required case conferences

Qualifications:

• Current, active Physical Therapist license in good standing
• At least 1 year of recent home health PT experience
• Proficiency with Home Care Home Base (HCHB) preferred
• Demonstrated OASIS assessment experience
• Solid problem-solving and clinical assessment skills
• Reliable personal transportation, valid drivers license, and appropriate auto insurance
• Able to independently manage a field-based schedule and caseload
• Excellent organizational and communication abilities
