Physical Therapist Home Health Washington, North Carolina

A rewarding opportunity is available for an experienced Home Health Physical Therapist to provide comprehensive patient care in a field-based setting. This contract assignment offers the chance to support patient independence and improve quality of life by delivering evidence-based therapy services in their own homes.

Schedule & Assignment Details:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM to 5:00 PM
• Caseload of approximately 2532 points per week (typically 57 patient visits daily)
• Daily travel to patient residences; mileage reimbursement provided
• Utilization of Home Care Home Base (HCHB) electronic documentation

Key Responsibilities:

• Conduct thorough physical therapy assessments, including OASIS evaluations at all required points of care
• Develop and implement tailored therapy plans aimed at enhancing mobility, strength, balance, and functional safety
• Deliver skilled interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies
• Evaluate patient progress, update treatment plans, and effectively communicate changes to the physician and interdisciplinary team
• Offer patient and caregiver education on safe mobility, exercise routines, home safety, and use of assistive devices
• Accurately complete timely clinical documentation using HCHB
• Actively collaborate with RNs, OTs, SLPs, medical social workers, and aides to ensure cohesive care
• Participate in case conferences and continuing education as required
• Adhere to all regulatory, agency, and Medicare guidelines

Qualifications:

• Current PT license (in good standing) within the state
• At least one year of recent field home health physical therapy experience
• Proficiency with HCHB preferred; OASIS experience required
• Strong assessment, clinical judgment, and documentation skills
• Reliable transportation, valid driver's license, and auto insurance
• Excellent time management, communication, and organizational abilities
