This contract opportunity is ideal for an experienced and compassionate Home Health Physical Therapist (PT) who thrives in a field-based, patient-centered role and values autonomy.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including Start of Care, Resumption, Recertification, and Discharge OASIS assessments as needed.
• Develop and individualize plans of care to address functional mobility, balance, pain management, strength, and safety.
• Implement skilled interventions such as therapeutic exercise, gait training, transfer training, neuromuscular re-education, and fall prevention.
• Educate patients and caregivers on safe home exercise programs, assistive device usage, and safety precautions.
• Document all care provided accurately and promptly using the Home Care Home Base (HCHB) electronic record system.
• Collaborate with the multidisciplinary healthcare team (including registered nurses, OTs, SLPs, MSWs, and HHAs) to ensure effective, coordinated care.
• Participate in interdisciplinary meetings, case conferences, and ongoing professional development activities.
• Adhere to agency protocols and regulatory requirements, including Medicare home health guidelines.

Qualifications:

• Active Physical Therapist (PT) license in good standing in North Carolina.
• At least 1 year of recent home health physical therapy experience (field-based preferred).
• Required proficiency with OASIS; experience with HCHB preferred.
• Strong clinical assessment, organizational, and time management skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently and adapt to a flexible, mobile work environment.
