Physical Therapist Home Health Marianna, Arkansas

Deliver impactful care directly to patients in their homes as a Home Health Physical Therapist. This contract role calls for a skilled, compassionate clinician ready to help individuals enhance their mobility, balance, and independence. You will work closely with patients and interdisciplinary teams, taking responsibility for evaluation, intervention, and education in a field-based setting.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Qualifications:

• Active Physical Therapist (PT) license in good standing
• At least one year of recent Home Health Physical Therapy experience (field experience preferred)
• OASIS experience required
• Proficiency in Home Care Home Base (HCHB) preferred
• Strong clinical assessment and decision-making abilities
• Reliable transportation, valid drivers license, and auto insurance
• Self-motivated, organized, comfortable working independently
• Excellent communication and organizational skills

Core Responsibilities:

• Conduct comprehensive physical therapy assessments, including OASIS evaluations for various stages of care
• Develop and implement individualized treatment plans targeting functional goals
• Administer therapeutic interventions such as exercise, gait training, and fall prevention
• Monitor patient progress, adjust plans as needed, and communicate changes to the care team
• Educate patients and caregivers on exercises, mobility strategies, and safe use of assistive devices
• Accurately document care in the Home Care Home Base (HCHB) system
• Collaborate with nurses, therapists, social workers, and aides for coordinated, patient-centered care
• Attend interdisciplinary meetings and engage in ongoing professional development
