PRN Physical Therapist at COTTONWOOD HEALTH AND REHABILITATION

As a Physical Therapist, you will provide compassionate, evidence-based care to help residents regain strength, mobility, and independence. You'll evaluate, develop, and implement individualized treatment plans in collaboration with other care professionals. This role is critical to enhancing residents' quality of life within our skilled nursing and rehabilitation centers.

Key Responsibilities

  • Perform comprehensive physical therapy assessments.
  • Design and implement individualized treatment plans.
  • Provide direct patient care and monitor clinical outcomes.
  • Collaborate with physicians, nurses, and interdisciplinary teams.
  • Educate residents, families, and caregivers on treatment goals and home programs.
  • Maintain accurate clinical documentation in compliance with regulations.
  • Supervise Physical Therapist Assistants (PTAs), as appropriate.

Qualifications

  • Graduate of an APTA-accredited Physical Therapy program.
  • Active Physical Therapist license in the state of employment.
  • Strong clinical judgment and communication skills.
  • Experience in a skilled nursing, rehab, or geriatric setting preferred.
  • Ability to remain composed and responsive in dynamic healthcare environments.
