Occupational Therapist

Key Responsibilities

• Evaluate and assess clients' physical and cognitive abilities and develop personalized treatment plans.

• Provide therapeutic interventions aimed at improving clients' ability to perform daily activities.

• Educate clients and their caregivers on adaptive equipment and home modifications.

• Collaborate with interdisciplinary team members including physical therapists, nurses, and social workers.

• Document clinical findings and maintain accurate, timely records in compliance with Medicare and company standards.

• Participate in case conferences and contribute to the overall care planning process.

• Perform OASIS assessments

Qualifications

• Licensed Occupational Therapist (OTR/L) in the Commonwealth of Virginia.

• Graduate of an accredited Occupational Therapy program.

• Minimum of one year of clinical experience (home health experience preferred).

• CPR certification required.

• Strong communication, organization, and interpersonal skills.

• Must have a valid driver's license, reliable transportation, and proof of auto insurance.
