Key Responsibilities
Evaluate and assess clients' physical and cognitive abilities and develop personalized treatment plans.
Provide therapeutic interventions aimed at improving clients' ability to perform daily activities.
Educate clients and their caregivers on adaptive equipment and home modifications.
Collaborate with interdisciplinary team members including physical therapists, nurses, and social workers.
Document clinical findings and maintain accurate, timely records in compliance with Medicare and company standards.
Participate in case conferences and contribute to the overall care planning process.
Perform OASIS assessments
Qualifications
Licensed Occupational Therapist (OTR/L) in the Commonwealth of Virginia.
Graduate of an accredited Occupational Therapy program.
Minimum of one year of clinical experience (home health experience preferred).
CPR certification required.
Strong communication, organization, and interpersonal skills.
Must have a valid driver's license, reliable transportation, and proof of auto insurance.
