POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in an acute rehab setting.

MINIMUM REQUIRED QUALIFICATIONS - CA RN license; BLS; 1+ year acute case manager experience

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - Mon-Fri with a weekend rotation

SYSTEMS - Meditech

START DATE - ASAP
