Job Summary: The Occupational Therapist shall screen, evaluate, re-evaluate, discharge plan and make revisions in any established plan of care as supervised by the Physician

Responsibilities/Requirements:

Screen all residents in the facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Occupational Therapy intervention is indicated.

Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.

Reassess patients and caseload every 30 days to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.

Supervise treatment implementation carried out by Certified Occupational Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.

Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.

Educate staff, family, and care givers as indicated.

QualificationsQualifications:

Bachelor's, Master’s or Doctorate degree in occupational therapy from an accredited program.

Certification and licensure requirements as established by state and national board associations.

Active state license

Excellent organizational and time management skills

Excellent written and verbal communication skills

Current working knowledge of Functional Pathways policies and procedures

Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
