Home Health Occupational Therapist - Newburg, MO

This contract Home Health Occupational Therapist opportunity spans 13 weeks, providing comprehensive care to individuals who need support regaining independence, safety, and functional abilities within their home environment.

Schedule:

• Full-time, Monday through Friday, 8:00 AM 5:00 PM

Primary Responsibilities:

• Complete thorough occupational therapy evaluations, including OASIS assessments as needed for Start of Care, Resumption, Recertification, and Discharge
• Design and implement tailored treatment plans to address ADLs, IADLs, upper extremity function, cognition, and safety
• Deliver skilled therapeutic interventions such as self-care training, adaptive equipment instruction, and environmental modifications
• Determine the need for and recommend durable medical equipment to optimize patient independence
• Monitor progress, modify treatment as appropriate, and communicate updates to physicians and the care team
• Educate patients and caregivers on fall prevention, home safety, compensatory strategies, and equipment use
• Maintain precise, timely documentation using Home Care Home Base (HCHB)
• Work collaboratively with nurses, PTs, SLPs, social workers, and aides
• Participate in interdisciplinary meetings and ongoing professional development

Desired Qualifications:

• Active Occupational Therapist license in the state (in good standing)
• At least one year of recent Home Health OT experience (field experience highly preferred)
• OASIS assessment experience required
• Proficiency with Home Care Home Base (HCHB) preferred
• Strong clinical, time management, and organizational skills
• Reliable transportation, current drivers license, and valid auto insurance
