Home Health Physical Therapist (PT) - Lake City, SC

Deliver essential care and support as a Home Health Physical Therapist, providing skilled therapy to patients in the comfort and privacy of their homes. In this contract role, your expertise will be instrumental in aiding individuals on their road to greater mobility, independence, and improved quality of life.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice
• Minimum of one year of recent field-based Home Health PT experience
• OASIS assessment experience required
• Familiarity with Home Care Home Base (HCHB) electronic documentation preferred
• Strong clinical decision-making, communication, assessment, and organizational skills
• Must have reliable transportation, a valid drivers license, and current auto insurance
• Capable of maintaining a flexible and independent schedule

Primary Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS as necessary
• Develop, implement, and continually adjust individualized therapy plans
• Deliver interventions focused on mobility, strength, balance, pain management, and fall prevention
• Educate patients and caregivers about exercises, use of equipment, and home safety
• Keep timely and accurate clinical records using HCHB
• Collaborate with other clinicians to foster coordinated patient care and attend interdisciplinary meetings
• Adhere to clinical standards and comply with regulatory and agency guidelines
