Physical Therapist Home Health - Florence, South Carolina

A skilled and compassionate professional is needed to provide physical therapy services to home health patients, helping individuals regain strength, mobility, and independence in the comfort of their own homes. This is a full-time, 13-week contract opportunity in a field-based setting, focusing on evidence-based therapeutic interventions and patient-centered care.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Qualifications:

• Valid and active Physical Therapist (PT) license in the applicable state, in good standing
• Minimum one year of recent home health physical therapy experience; field experience highly valued
• OASIS assessment proficiency required
• Working knowledge of Home Care Home Base (HCHB) preferred
• Reliable transportation, current drivers license, and active auto insurance
• Independent work ethic with strong clinical, communication, and documentation skills

Primary Responsibilities:

• Conduct thorough PT evaluations and OASIS assessments where appropriate
• Formulate and implement individualized care plans targeting mobility, strength, balance, and pain management
• Administer targeted interventions: therapeutic exercise, gait/transfer training, neuromuscular re-education, and fall prevention strategies
• Adjust treatment plans based on ongoing reassessment and patient progress, collaborating with physicians and interdisciplinary care teams
• Provide education to patients and caregivers on exercise safety, assistive devices, and home exercise routines
• Complete accurate, timely documentation using Home Care Home Base (HCHB)
• Attend case conferences and contribute to interdisciplinary meetings as required
• Commit to compliance with agency policies and home health/Medicare regulations
