Home Health Physical Therapist in Timmonsville, SC

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice
• At least 1 year of recent home health physical therapy experience preferred, with strong field skills
• Proficient with OASIS assessments and experienced in Home Care Home Base (HCHB) electronic documentation (preferred)
• Solid clinical decision-making and functional assessment abilities
• Reliable transportation, valid drivers license, and insurance required
• Able to work independently and efficiently manage a flexible, field-based caseload
• Excellent communication, organization, and time management

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations in patient homes, including OASIS assessments as applicable
• Develop and execute individualized care plans focused on functional mobility, strength, balance, and patient safety
• Deliver therapeutic interventions (exercise, neuromuscular re-education, gait and transfer training, fall prevention)
• Monitor patient progress, modify treatment plans as needed, and communicate changes to physicians and team members
• Educate patients and caregivers on exercises, safety, home programs, and assistive equipment use
• Maintain timely, accurate documentation using HCHB
• Collaborate closely with nurses, occupational therapists, speech therapists, and other team members for coordinated care
• Attend case conferences, interdisciplinary meetings, and continuing education programs as required
• Adhere to all agency and Medicare home health standards and protocols
